Confidentiality Agreement

Please read this agreement carefully before submitting your application.

I understand that during my volunteering role with Health Connections, I may hear, see, receive, or have access to confidential information that must not be disclosed to any person other than in the circumstances outlined below.

I agree, both during and after my time as a volunteer, to respect and maintain the confidentiality of any personal or sensitive information shared with or obtained through Health Connections. This includes information held in paper records, electronic systems, emails, messages, conversations, or any information acquired verbally.

I undertake not to disclose any information relating to individuals receiving support or services from Health Connections, including but not limited to:

• Names, addresses, contact details, or other identifying information.

• Details of any enquiry, referral, or request for support.

• Details of any advice, information, guidance, or assistance provided.

• Any personal, health, family, financial, or other sensitive information learned in the course of my volunteering.

I understand that confidential information may only be shared:

​

• With the express permission of the person concerned.

• Within Health Connections where this is necessary to provide appropriate support or services.

• With my designated mentor, supervisor, or a member of staff where this is necessary to support my volunteering role.

• Where there is a safeguarding concern, risk of harm, risk to life, or where disclosure is required by law. In these circumstances, information may be shared with the appropriate person, organisation, or authority in line with Health Connections' safeguarding policies and procedures.

I understand that Health Connections operates a safeguarding first approach. While personal information will be treated confidentially wherever possible, confidentiality cannot be guaranteed where there are concerns about the safety or wellbeing of a child, young person, adult at risk, volunteer, member of staff, or another person.
