How to get signposting support

Contact our team and they will arrange a time to meet to have a chat about what you would like to do, your interests and how we can support you.


Our aim is to make you feel empowered to do things which will enable you to live a happier, healthier and more fulfilled life. Community Connectors provide short term support, helping you to take the first steps to finding the support you need and making sustainable changes.


We also signpost to other agencies/resources if this would be more appropriate.

What is signposting support?

Our Community Connectors can listen to your story and provide information about local services such as support groups and activities.


This could include:

  • Identifying the support you need to to live a happier, healthier and more fulfilling life.

  • Connecting you with a support group, club, activity or a health and social care service.

  • Helping you to access transport to get there.

  • Helping you to navigate systems and access support and services that matter to you.

Become a Community Connector

If you think you have the skills needed to become a Community Connector and volunteer in our offices or Talking Cafes, we would love to hear from you.

Community Connectors are members of staff and volunteers who have advocacy and 1:1 signposting support skills. They are DBS checked and undertake training which includes safeguarding and boundaries.

 

Community Connectors connect people to support that helps build knowledge about support available, develop skills and confidence and provide health information, advocacy or just a supportive ear which help people improve their health and well-being or manage their long term condition.
