1999 – Founded as Health Information Guernsey, providing trusted health information and guidance.

2010 – Awarded the Queen’s Award for Voluntary Service in recognition of its impact.

2017 – Embarked on a strategic transformation in response to the island’s Health and Social Care policy and evolving wellbeing needs.

2018 – Incorporated as Health Connections LBG to strengthen its structure and sustainability.

2019-2025 - Implementing its strategic and business plans and achieving 67% self sufficiency through 2 charity shops.

I'm blessed to have a values-led job supporting our amazing team of staff and volunteers, helping people feel less alone, access the support they need, and be part of something bigger. It’s inspiring to witness so much kindness and see lives truly changed.
Being part of this team lets me turn compassion into action every day. And our shops? They truly are gems and make my heart sing, spark creativity, and bring delight and calm to the people we serve.

Our Pledge for the Future
Improved health, care and wellbeing for all
We will support health, care, and wellbeing professionals, along with community groups, to work collaboratively. This will enable easier self-referral and cross-referral to health and wellbeing support.
Partnership Approach
With people at the heart of all we do, we will foster a culture of community-based, intergenerational, and interdisciplinary trust, participation, and care.
People-Centred Support
We will make it easier for people to navigate support and care of their own choice, with a focus on improving access for the most vulnerable.
Systems Thinking
We will work across public, private and charity sectors — including Health & Social Care, Employment & Social Security, Environment & Infrastructure, Housing, Education and the Arts in service of better outcomes.
Prevention & Early Intervention
We will champion and support the work of our Directory Partners, Primary, Secondary, and Community Care.

We will help the Bailiwick of Guernsey move towards becoming a global exemplar for health and wellbeing by aligning with the Social Model of Care, building social and environmental capital, and striving to remain a self-sustaining charity.

We will demonstrate our impact clearly, while continually adapting and improving our offer in response to the needs of our partners and the people we serve.
